Is there a php script (or other type) that would automatically fax a form submitted via email?
Has anyone heard of this, or would it involve software/E-fax type of interaction?

well, I haven't heard of it, and anyway, I'd guess php to be the wrong solution for the problem.
You'd need a server with a fax software for once, then a flexible fax software and the bit that takes it from the webform and hand's it over to the fax software... nothing to difficult.
Alternative would be to find a mail-to-fax gateway somewhere, and just sent the forms as mail to said gateway. That would be doable in php. You'll need flooding control for sure, after all, you don't want to be swamped with faxed because some lifeless punk hit the 'submit' button a gazillion times...

Nethermind: It is certainly doable if they want to pay but doing it for free is a little trickier.

The Phone Company's system can be used for some free faxing (you could either reverse engineer their forms or just construct an email and send it) but coverage isn't 100% and faxes to each number are limited to stop people using the service for business/spamming.

Thanks for the tips. I am creating an online application form for a real estate company, and the owner doesn't really like to check his email. He'd rather receive the applications via fax automatically.

He recently went to a real estate convention where somebody showed him this software application that automatically buzzes your pager when you receive an application from the website, and I think it really had an impact on him. I don't think he would want this fax service enough to pay a lot of money for it or to pay for expensive software and setup. I told him that I'd search around to find something. After an afternoon of Googling, I just found the usual E-Fax stuff. I guess I could have him pay me for the hassle of sending them from an inbox to his fax machine each morning, but it seemed like a lo-tech solution.
