Ok, basically, I want something just like this...

But for some reason, I am not getting it to work right... I am trying to get a form set up that will allow me to easily get trouble call codes, but I am not having much success. I want the exact kind of thing they have there, but I want a submit button that will translate the descriptions into the actual codes... But I have only been able to get the 3rd level description to pop up

I have been googling for hours to no avail, I'm about to head back into cyberspace to look again... If anyone has any ideas, let me know.

Not entirely sure where this breaks down on the server side or client.

That is what I have done so far. The biggest problem is that I have to add each item in by hand, as can be see here:

And I have only done one freaking section. I have 13+ left to go, I have been trying to get it to where each select will specify a seperate code so that I can reuse some of the 2nd and 3rd level codes... It would be great if I could just keep it client-side so that I can put it on a CD so it won't have to be downloaded to each office locations server. But I don't know, I have run into a brick wall with this stupid thing.
