How do you hide one image in another which shows up when the image is selected?

This seems to be a popular fad at the moment and is explained in the resources below:

One supplementary question:

Surely it must be possible to do something interesting and artisitc with this - most of the examples I've seen (other than this) just hide pictures of naked women in rather innocent scenes.
