HELP!!

I have never really written a "formal" invoice for graphic design. I have done a job for a large company in my area. It was a spontaneous contracted job and they needed it quick. They want an invoice and I was wondering if someone could give me some words of wisdom! If possible, a word for word demonstration would be a HUGE help! I have already done the work...what do I need to cover my behind?

If you have 'word' I'm pretty sure it has a few invoice 'templates' ... just fill in the blanks... or
look at some of these and see if there's something suitable.
