Do you mean in the Swatches palette or an Index mode image?

If you are talking about the Swatches, you can add custom colours at the end by clicking there. Other than that, I don't know much about custom Swatch palettes.

If you are talking about the other, just hit Image > Mode > Colour Table (if already in Index mode) and fiddle to your hearts content (which will be reflected in the Swatches palette).

You can also make your own completly custom swatches and save them as.. well whatever you want to call them really...

I've got some nice swatches saved for skintones and metals in sunlight/bluelight (the two colour lights I most often use..). It really saves a lot of time when your using PS as a painting tool.. I usualy make a swatch of base colours (not primary colours) and/or grey tones when I first start any kind of painting.

It also comes in handy when I'm doing web work. I can save all of the colours (with hex codes) in the swatch so If I need an image to blend into the background of the page/table or if I want to colour something into the page colour scheme etc etc...

Just play around with the different options you'll fugure it out.. if your using ps6 remember that little ">" arrow.. important..

I'm sure there are even more uses for the swatches when your doing print work but I hardly ever do print work so I would know. However I think the ability to save the exact pantone colours your working with whould be a huge plus on it's own....
