There used to be a filter, I think it was called "3d-transform"
where you could make up spheres and boxes to place image-element
on but I can't find it in Photoshop CS!

Is it still there? And in that case...where?!? )

Until the 7.0 ( I don't have CS ), it's been in Filter > Render > 3D Transform.
But, if you can't find it, you could still to the transformations by hand, via the displace filter ( Filter > Distort > Displace ). As explained by warjournal in Power Distort

Hmm, yeah. The problem is I want to place a logo
on a baseball-cap but the cap is photographed slightly
from the side so I need to both give the logo some perspective
as well as bend it "around" the cap..

And I think 3d-transform had been great for that here...

3D Transform is shipped with photoshop CS it's just not install by default. It is locate along with a few others in the goodies folder.

There are tons of useful items in the goodies folder, including many older file formats IFF or less used formats like RLA and JPG 2000 and HSL plug-ins, some a whole slew of option plug-ins for trouble shooting and system teaking plug-ins.

Another option is to use the ball cap/shirt/etc AS the displace map.
Most things wrap pretty well around wrinkles and perspective.

I've seen a video where a "PhotoShop Guru" put a sticker on a tshirt. He used the chanel with the widest range as Mask, and via the Liquify menu he distorted the sticker in X through the mask, then inverted the mask and distorted the sticker in Y. Finally he played with the blending modes to brink back the shadows and highlights of the tshirt on the distorted sticker and voilà.

[edit] In the flyer's case, I would slightly adjust the perspective of the logo to match that of the baseball cap, and then distort the logo with any of the methods mentionned here [/edit]
